His sister. Isami has a serious case of siscon, practically a disease. He categorizes women as ‘Manami’ and ‘the others’, so he has reached a summit where even doctors would throw in the towel.

Hakuto Kunai[]
His relationship with Kunai is not good or bad. Though it is a business relationship for the most, Isami is grateful to Kunai for rescuing him and his sister long ago.
Unknowing that Akira Oono is now in control, Isami is often used by him to strategize; allowing him to think Kunai is simply only allowing portions of a grander scheme to be revealed. He is trusted by Kunai to look after children, which Isami does well despite his wisdom defect.
Yu Kirino[]
She scares him due to her sadism and he is very afraid of her, preferring not to think of the horrors she commits in her hospital. Even if he were dying he would not accept being healed by her for fear of what she would do to him. Despite that, the two of them can have a casual conversation and Hakuto notes that they get along well with each other. Isami prays that the personality shift she experienced in the New world is permanent.
